Group Leader Registration Form
The California Gospel Ministries(CGM) is looking for hard working people to join our CGM
family as Group Leaders.  The policies and procedures of a CGM Group Leader are listed
below.  The CGM have a great incentive program.  Our Group Leaders have the opportunity to
be rewarded with trips as well as  Cruise rewards.  Once you have read the policies and
procedures for the CGM Group Leadership program and you want to be come a CGM Group
Leader, complete the form below.  Once we receive your completed Group Leader
Registration Form, we will contact you with additional information.



RETREAT

GROUP CONSISTS OF A MINIMUM OF 12 - 15 FULL PAID HOTEL ROOMS OR 24 - 30 PEOPLE.  
THE INCENTIVE:

ONE (1) COMPLIMENTARY OCEAN VIEW  CABIN FOR YOU AND ONE GUEST ON  CARNIVAL' S
PARADISE CRUISE SHIP, JANUARY 25 - APRIL 14.   NOTE:  AIR TRAVEL IS NOT INCLUDED IN THE
INCENTIVE.

A GROUP CONSISTING OF A MINIMUM OF 8 - 11 FULL PAID HOTEL ROOMS OR 16 - 20 PEOPLE,
THE INCENTIVE:
(ONE (1) COMPLIMENTARY WEEKEND FOR TWO (2) AT THE DAYS IN HARBOR VIEW HOTEL IN SAN
DIEGO, CA.    NOTE:  AIR TRAVEL IS NOT INCLUDED IN THE INCENTIVE.

GROUP CONSISTS OF A MINIMUM  OF 5 - 7 FULL PAID HOTEL ROOMS OR 10 - 14 PEOPLE
ONE (1) COMPLIMENTARY GOURMET LUNCH FOR TWO (2) ABOARD THE NAPA VALLEY WINE TRAIN.

CRUISE

A GROUP CONSISTS OF A MINIMUM OF 16 (SIXTEEN) FULL PAYING PASSENGERS, OR 8 CABINS. THE
THIRD AND FOURTH PERSON(S) IN THE SAME CABIN ARE NOT INCLUDED IN THE CALCULATIONS
FOR THE INCENTIVE.  NOTE: AIR TRAVEL IS NOT INCLUDED IN THE INCENTIVE.  THE GROUP
LEADER IS RESPONSIBLE FOR PAYING HIS/HER OWN AIRFARE, PORT CHARGES, TAXES AND
GRATUITIES.


Incentive awards are as follows:

16 passengers - One complimentary passenger cabin fare.
32 passengers - Two complimentary passenger cabin fares.
48 Passengers - Three complimentary passenger cabin fares.
64 Passengers - Four complimentary passenger cabin fares.*

*A maximum of four complimentary passenger cabins fares can be earned.

ALL GROUP LEADERS

Group Leaders are required to confirm their reservations with an application and deposit for the
specific room or cabin category desired.  Group Leaders are required to make monthly payments as
outlined for their respective retreats or cruise.  Once you have reached the required number of
group members for a complimentary incentive program or passenger fare, you will be given
necessary information on the cruise, Days Inn and Napa Valley.  OR You can choose to be refunded
the cruise fare only, based on the average category booked by your members.  Group Leaders may
upgrade their cabin category for an additional charge (if applicable).

Please complete and return the Group Leader Registration email or mail.  Once you submit a Group
Leader Registration Form, you are acknowledging that you will adhere to the CGM group program
policies and procedures.  When your form is processed, you will be assigned a Group Leader  
Identification Number

Reservations are taken on a first come, first served basis.  NO CABINS WILL BE HELD WITHOUT A
DEPOSIT.

Please inform your group members that your full name or group number MUST ACCOMPANY their
reservations during the initial registration process in order for you to receive credit.
