The California Gospel Ministries Conditions:
A non refundable deposit of $50.00 per person is due with your completed Reservation Form. Additional MONTHLY
PAYMENTS of $75.00 per person is due every 45 days until paid in full. Three/Four people (Triple/Quad)in a cabin the
initial deposit is $250.00 person is due with your completed Reservation Form, and $75.00 per 3rd/4th person due in
45 day increments until paid in full. Suites deposits are $300.00 per person due with your Completed Reservation
Form and $100.00 per person, per month due every 45 days. All cabins must be paid in full by October 15, 2012 (THIS
DATE IS FIRM) ALL REFUNDABLE PAYMENT will be paid twenty-one days after date of cruise. We at The California
Gospel Ministries strongly recommend that you purchase Trip Cancellation Insurance to protect your travel
investment. The cruise line offers insurance. Ask your agent for cruise line quote or you can go to: www.travelguard.
com for a quote. If you are not purchasing the Trip Cancellation Insurance, a Travel Insurance Acceptance/waiver
form must be completed and forward to your travel agent. Persons with pre-existing health conditions must
purchase insurance at time of booking or within 15 days of booking.

Your Reservation Form must be completed in its entirety. We will mail information in regard to the Payment Program with your invoice for initial
payment.

Stateroom features the luxury of your Comfort Bed, soft carpeting, indirect lighting, lots of closet space and individual climate
control.   Depending on your preference, you can be on a higher deck, closer to the pool area, more conveniently located to your
dining room, steps away from the duty-free shops or — if you’re a parent— closer to the children’s facilities.   We also feature
connecting staterooms for families and groups of friends. These staterooms are limited, so make your choice ASAP.

Dining preferences (time, companions) No requests are guaranteed. By popular request, all our
Dining Rooms are now Smoke-Free. Your table assignment will be confirmed at embarkation. Dining

You can also enjoy a continental breakfast in your stateroom (until 10:00 AM or a limited breakfast
menu on deck Lunch and dinner are also available bistro-style on the Lido Deck (the seaview bistro is
not available on the last night of the cruise).

non-refundable or transferable.
Also I understand that name changes are allowed and are subject to a fee depending on the change
date. I further understand that if my roommate(s) cancel, my cabin rate will change. The California
Gospel Ministries, the cruise line, or tour operator reserves the right to cancel any guest who fails to
comply with the payment schedule and the cancellation fee will be charged according to the
cancellation policy. A late fee will be accessed to any account owing a balance after the final payment
date.
Returned Check fee is $50 per check. I am aware that Rates, scheduled events, activities, speakers,
special guests, entertainers, sponsors, host or cruise itinerary are subject to change and may not be
used as a reason for cancellation.
